dTS application checklist

  • DTS Application Form

  • DTS Registration Fee: A non-refundable registration fee of $35 is to be forwarded with your completed application. The fee is required in order to process your application.

  • Pastor’s Reference: Please send this form to your pastor or spiritual leader for completion. Then have them email us the completed form.

  • Employer or Teacher’s Reference: Please send this form to an employer or teacher for completion.Then have them email us the completed form. 

  • Friend Reference: Please send this form to a friend for completion. Then have them email us the completed form.

  • Confidential Health Form: Please have a physician complete this form.

  • Passport size picture: You can send your photo via email at www.ywamlongbeach.org or mail to YWAM Long Beach.

  • Once we review and pray over your application, we will email you with your acceptance package. *Please do not purchase airfare or accommodations or apply for your visa, until you receive your acceptance letter.
